    At Quest Inc, the Licensed Practical Nurse (LPN)- Appointment Nurse schedules client medical appointments. Ensures transportation for clients to and from appointments. Accompanies clients on appointments or assigns appropriate staff to accompany client. Completes other duties as assigned.


    • Schedules and attends medical appointments with clients. Transcribes new orders from consulting appointment and verifies with Medical Director. Transcribes new orders onto MAR/TAR, fax/call orders to pharmacy. Gives report to floor nurse regarding new orders received on appointment or any changes made that day.
    • Schedules follow-up appointments, annual appointments (dental cleaning, dentist visits, annual in house eye exams, podiatry visits, psychiatric appointments) as well as specialty consultants that vary with each client.
    • Requests results of diagnostics and follows up post appointment
    • Rounds with physician on his weekly visit, as well as carrying out all new orders received that day (transcribing orders, faxing orders to pharmacy)
    • Oversees annual physicals are completed each month
    • Schedules labs pertaining to outside appointments; draws up lab requisition, schedules a date and time for the lab draw to take place and transcribes onto MAR
    • Completes Infection Control rounds monthly.
    • In the event of a call off, works the floor in place of the absent nurse.
    • Complies with nursing policies and procedures to ensure nursing standards are met. Promotes optimal nursing interventions.
    • Administers medications and treatments per physician’s orders and in accordance with state and federal guidelines.
    • Must be able to administer as well as count controlled substances.
    • Performs nursing evaluation of persons served health status daily.
    • Monitors persons served for seizures, medication side effects, signs and symptoms of illness, and provides necessary nursing interventions.
    • Assists in monitoring of clients.
    • Informs RN, Health Care Coordinator (HCC) or Director of Nursing (DON) and/or physician of health status change/accidents and injuries of persons served.
    • Completes all required documentation including but not limited to assessments, nurses notes, tracking forms, incident reports, monthly/quarterly reports, etc., along with quality assurance reviews, filing, and purging medical files.
    • Assists with providing nursing services in group homes as needed.
    • Demonstrates ability to administer IV medications and IV solutions as ordered by physician and within appropriate nursing regulations.
    • Participates in interdisciplinary, habilitation planning, and other meetings as requested.
    • Demonstrates understanding of the persons served training programs, behavior plans, and service goals, as developed during annual habilitation plans.
    • Ensures Safety and Infection Control Policies and Procedures are followed.
    • Ensures all emergency procedures are followed and communicates accidents reports, incidents of abuse, neglect, and exploitation in accordance with Zero Tolerance policy.
    • Maintains current CPR certification and current LPN License.
    • Adheres to all Quest’s policies and procedure to ensure client medical needs are addressed.
    • Reports to work during inclement weather to ensure client health needs are met.
    • Provide supervision of nurses and/or DSPs during appointments


    • Eligible to legally work in the United States
    • Be at least 18 years of age
    • Successfully complete a background check and drug screen
    • Ability to read, write, and speak English
    • Ability to thrive in a team environment
    • Ability to accurately complete nursing documentation
    • Ability to use an Electronic Medical Record system
    • Valid FL Driver’s License with a clean driving record
    • Ability to provide proof of educational achievement or required certifications such as a high school diploma, GED, college degree, advanced degree, or medical credentials
    • Must have a valid unrestricted license as LPN in the state of Florida
